Fracture-Dislocation right shoulder
This image shows a dislocated shoulder with a fracture through the surgical neck of humerus. The patient is at high risk of axillary nerve injury. The axillary nerve supplies deltoids but this is difficult to test in these conditions - luckily it also supplies an area of skin over the shoulder known as the regimental badge - this must be tested before and after any procedure involving the shoulder.

Common Fractures and Dislocations in Children
Fracture of the clavicle: Can occur after a fall on an outstretched arm (more likely to cause a Colles’ fracture in the old). Collar and cuff sling for 3 weeks, internal fixation only if there is non-union. Beware neurovascular injury and pneumothorax.

Developmental Dysplasia of the Hip
Developmental dysplasia of the hip (DDH, formerly known as congenital dislocation of the hip) describes any abnormality found in the neonatal hip joint. The degree of abnormality varies considerably. The problem may arise from structural defects in the head of the femur, the acetabulum or surrounding structures. Approximately 1 in 1000 births are affected and it is more common in the left hip.

A 59 year old woman presents to the emergency department with an isolated anterior dislocation of her left shoulder after a fall. Other than controlled asthma, she is healthy. Her last meal was four hours before the injury. After adequate analgesia using intravenous fentanyl, and despite an initial attempt to reduce her dislocation using relaxation techniques, it becomes evident that she needs sedation to complete the procedure successfully.
